We are seeking an Outreach & Engagement Case Manager to work with us remotely from anywhere within the US. Mindoula's teams of case and care managers, supported by technology including a mobile engagement app, proprietary psychometrics, and predictive analytics, are the key to its success. Mindoula expects to expand rapidly across multiple business lines in multiple states and is seeking an experienced Outreach & Engagement Care Manager.

This Outreach and Engagement Care Manager will have top-notch communication and customer service skills, an exceedingly pleasant phone manner, the ability to work as part of a team as well as independently, and a high degree of professionalism. Comfort in speaking to people with behavioral health needs and Crisis Intervention training is a must. The Outreach and Engagement Care Manager will have experience in assessing risk and triaging patients to various levels of care according to stated needs and validated assessments including the M3 and Columbia-Suicide Severity Rating Scale Scale. Additionally, the Outreach and Engagement Care Manager will have experience verifying medical insurance benefits. Last, the Care Manager will be extremely organized, self-motivated, and at ease with a variety of computer programs including electronic health records and G-suite.

How you'll contribute:

  • Manage a caseload of pending patients to ensure timely outreach of referrals (within 2 business days) and appropriate and timely follow-up.
  • Adhere to programmatic metrics for the Outreach and Engagement team.
  • Provide appropriate education about Mindoula’s Collaborative Care Program and be able to answer questions regarding cost, insurance, and eligibility to best prepare patients for enrollment.
  • Support patients with insurance verification and when necessary, complete the insurance verification to confirm coverage.
  • For eligible patients, following agreement of enrollment, scheduling of intakes with Intake Care Managers.
  • For patients who are not eligible, ensuring necessary resources are provided and that the ordering provider is informed of the patient not enrolling.
  • Evaluate the behavioral health needs of a patient via validated assessments and interview questions and assess risk for self-harm.
  • Encourage patients to express feelings and discuss their behavioral health struggles and needs.
  • Produce high quality documentation that enables the patients to receive the best treatment possible.
  • Prepare and maintain all required records, reports, paperwork, and patients’ diagnostic records, maintaining confidentiality of patients at all times.
  • Accurately complete all case notes in a professional, timely manner.
  • Enter data into multiple locations in a timely manner without error.
  • Uphold professional standards as a representative of Mindoula Health.
  • Improve our program and work environment through constructive suggestions and show each patient respect and compassion without crossing therapeutic boundaries, infringing on ethical standards, or being drawn into unethical situations.
  • Work as part of the clinical team; promote teamwork and unity.
  • Use clear and accurate communication skills (both verbal and written) to communicate with patients over the phone and through email.
  • Respond positively to supervision and to recommendations for improved performance.
  • Work under high stress with multiple priorities while maintaining the ability to respond to patients and their support systems in a healthy manner.
  • Relate to patients in a nonthreatening manner that is not rude, impersonal, or degrading to the patient.
  • Other tasks as they arise.

Salary will be a combination of base and variable compensation, with significant maximum compensation potential for hitting targeted metrics.

What will make you successful:

  • Located on the west coast preferred, but not required.
  • Bachelor’s Degree in psychology, social work, or other behavioral health discipline from an accredited college or university
  • One to two years of experience in the behavioral health field and/or healthcare environment.
  • Customer service experience.
  • Crisis Intervention Training /Crisis Training
  • Requires extreme comfort with technology and ability to learn new systems, transition between technologies easily, and ensure data accuracy
  • Technologies include:
    • Google Office Suite - Docs, Sheets, Drive, Slides
    • Microsoft Office Suite - Word, Excel, PowerPoint
    • Web browser
  • Work experience with EHRs and ability to work with multiple simultaneously.

Skills Required:

  • Knowledge of medical and health terminology.
  • Relentless about prioritizing and following through on assigned tasks independently.
  • Superior time management and organization to complete daily tasks by the end of each day
  • Proficient in Data entry/database management experience with Microsoft Excel and other systems/applications.
  • Compliance: Ensures care is delivered according to applicable laws, regulations, policies and procedures, supporting the organization’s corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all organizational investigations and proceedings. Ensuring health information management requirements are met.
  • Ability to exercise strict confidentiality and sound judgment with respect to sensitive patient information, maintaining the integrity of patient records at all times.
  • Relationship Management: develop, maintain, and strengthen partnerships with others inside or outside the organization.
  • Problem Solving: Identifies needs and takes independent action to implement change when and where it is needed; results oriented. Ability to plan, organize, manage time, and prioritize multiple tasks and assignments. Use effective follow through.
  • Communication: Excellent listening, verbal and written communication skills. Clearly and accurately, relaying information to staff, business partners, stakeholders, and patients.
  • Ability to work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness.
  • Ability to provide case management in routine and crisis situations.
  • Excellent attention to detail and organizational skills.
